How to Apply for Observation

Students and individuals doing observational hours at Aultman are required to complete the following:

• Registration Form
• Student Acknowledgement Statement 
• Safety Packet and Quiz
• Two-step TB skin test or two consecutive years of a one-step and full documentation  

All forms must be turned in two weeks prior to the requested observation date. You will be notified by e-mail or a phone call if you are accepted to observe in the hospital. Remember that there is a limited amount of experiences for each day offered.
