Students and individuals doing observational hours at Aultman are required to complete the following:

Along with other Stark County hospitals, Aultman is encouraging vaccination of staff, students and volunteers. Those declining vaccination will be required to wear masks upon entry to all designated patient care areas. Individuals who receive their vaccinations from an outside provider must submit documentation to Employee Health Services.
All forms must be turned in two weeks prior to the requested observation date. You will be notified by e-mail or a phone call if you are accepted to observe in the hospital. If submitted documents are not complete with all required forms included, then the request will not be considered or processed. Thank you for complying with our hospital health and safety standards.
