Our Services

Home Health Care



Aultman's skilled Home Health Care team - consisting of registered nurses; physical, occupational and speech therapists; social workers; and home health aides - will come to your home after discharge to meet your health-care needs. Home Health Care services must be ordered by your physician.

Skilled nursing and therapy services are covered by Medicare and Medicaid, along with many private insurance carriers. For your peace of mind, we will gladly answer your insurance questions and submit all insurance claims on your behalf. Payment plans may also be arranged.

Should you require homemaker or companion services instead of skilled nursing, Aultman Woodlawn offers private-duty care through our Alternative Daily Living program.

Here are the Home Health Care services we provide:

Home Health Aide

  • Home Health aides are certified. They may assist the patient and family with personal care on an intermittent basis when there is a skilled service provided in the home in accordance with a physician's orders.
  • The Home Health aide, under the direction of the registered nurse or therapist, may provide the following services: bathing, hair care, grooming, dressing, toileting and range of motion.

      Social Services

    • Social services are available and can provide the patient with the following services: financial resources, community services referrals, counseling and placement assistance.

        Nutritional Services

      • A registered dietitian is available for consultation to staff and to patients. Services from the dietitian include: education for new or uncontrolled diabetes, Total Parenteral Nutrition (TPN), tube feedings and weight loss.
