Includes exercise planning, risk-factor modification, education and long-term follow-up
Heart Failure Support Group
Focuses on monitoring patient symptoms, offering education on activity, diet, medications and providing the appropriate follow-up
Arrhythmia/ICD Support Group
Provides patients and families with education and emotional support
Cardiac Risk-Factor Program
Offers one-on-one consultation with Heart Center nurse, who will determine individual's risk for heart disease
