Case Management
Case Management is responsible for coordinating a patient's care from admission through transition out of the hospital. Health care professionals including registered nurses and licensed social workers make up the Case Management team. Case managers work collaboratively and under the direction of a patient’s physician.
Upon a patient’s arrival to the hospital, Case Management provides support to the patient and his or her family as well as assuring that the hospital stay meets specific needs. In addition, case managers work with a patient's insurance company to determine coverage of services.
Case Management also develops comprehensive discharge and transition plans when leaving the hospital. With an expertise in patient care, case managers support the patient and family in lifestyle transition and proper care after discharge. Additionally, Case Management links the patient to appropriate resources in the community.
